阿尔茨海默症(Alzheimer's disease, AD)是一种严重的神经退行性疾病,临床上以记忆障碍、失语、失用、失认、视空间技能损害、执行功能障碍以及人格和行为改变等全面性痴呆表现为特征,病因迄今未明。该病现在已经成为威胁老人健康的四大杀手之一,极大地影响了个人、家庭乃至社会的发展。当前,已发现的AD 病理途径主要包括乙酰胆碱功能障碍,增加的β淀粉样(β-amyloid, Aβ)蛋白沉积,神经纤维的缠结,以及其他的与生物靶点相关的途径。研究发现可通过抑制β-淀粉样蛋白的聚集和沉淀;防止tau 蛋白的异常过度磷酸化;改善线粒体功能障碍,抑制神经元凋亡;抑制过度增殖活化的小胶质细胞及减少白细胞介素-1和肿瘤坏死因子-α的含量;减少氧化应激损伤障碍;调节胆碱能系统相关靶点;抑制谷氨酸受体的过度激活,减少谷氨酸引起的神经元毒性等方面预防和治疗阿尔茨海默症。

在AD 发病机制尚未完全研究清楚之前,针对 AD 的治疗只能是对症治疗,这并不能从根本上治愈 AD 患者。所以,找到可逆转疾病进程的药物才是攻克AD 这一疾病的关键。众多临床试验失败的教训提示单一靶点的治疗对于AD 这种复杂疾病很难奏效。而多靶点药物和鸡尾酒组方药物可能是未来AD 药物研发的一个重要方向。此外通过小分子化合物促进神经再生也可能是一种新的研发策略。
